  1. Hepatic Clearance predominates
  2. Half-Life: 1-3 hours (9-10 hours of affect)

V. Adverse Effects

  1. Background
    1. Adverse effects are common and limit use (other medication options are preferred when available)
  2. Arthralgias or myalgias (40%)
  3. Infusion site pain (40%)
  4. Nausea, Diarrhea or Vomiting
  5. Hyperbilirubinemia
  6. Rash or Pruritus
  7. Headache

VI. Drug Interactions: Metabolized by Cytochrome P-450 3A4

  1. Raises serum drug levels of substrates in 3A4 system
  2. Follow Cyclosporine levels when using concurrently

VII. Dosing

  1. Enterococcus faecium: 7.5 mg/kg IV q8 hours
  2. Severe Skin Infections: 7.5 mg/kg IV q12 hours
