IV. Dosing: Adult

  1. Herpes Zoster (Shingles)
    1. Adult: 500 mg orally three times daily for 7 days
  2. Genital Herpes
    1. Initial: 250 mg orally three times daily for 7-10 days
    2. Recurrent immunocompetent: 1000 mg orally twice daily for 2 days
    3. Prophylaxis: 250 mg orally twice daily for up to 12 months if more than 6 episodes per year
  3. Orolabial Herpes
    1. Recurrent: 1500 mg orally for one dose at onset
  4. HIV (orolabial or genital)
    1. Recurrent: 500 mg orally twice daily for 7 days
  5. Renal
    1. Adjust dose for Creatinine Clearance <60 ml/min
