- Capsules: 400 mg
- Chewable Tablets: 100 or 200 mg (contain Phenylalanine)
- Suspension: 100 or 200 mg/5ml and 500 mg/5 ml
- Stable at room Temperature for 2 weeks
VI. Dosing
- Adult (or child age >13 years, weight > 45 to 50 kg)
- Standard Dosing
- Take 400 mg orally divided once daily
- Gonorrhea
- Take 400 mg orally for single dose

- Child (age <13 years or weight < 45 to 50 kg)

- Give 8 mg/kg/day divided daily to twice daily
- Febrile Urinary Tract Infection
- Give 16 mg/kg orally on day 1, then 8 mg/kg/day daily for 14 days

- eGFR 21 to 60 ml/min (or Hemodialysis): Decrease usual dose to 75%
