IV. Dosing
- Adult (or child weight <40 kg)
- Give 1 to 2 grams IM or IV every 4 to 6 hours
- Use 1.5 to 2 grams IV every 4 hours in Osteomyelitis
- Child (weight <40 kg)
- Mild to Moderate Infection
- Give 100 to 150 mg/kg/day IM or IV divided every 6 hours
- Severe Infection
- Give 150 to 200 mg/kg/day IM or IV divided every 4 to 6 hours
- Mild to Moderate Infection
- Newborns (Weight >2 kg)
- Age <1 week
- Give 25 to 50 mg/kg IV every 8 hours
- Age >1 week
- Give 25 to 50 mg/kg IM or IV every 6 hours
- Age <1 week
V. Adverse Effects
- Interstitial Nephritis

Liver dysfunction
- Occurs with dose >12 g/day
- Monitor hepatic panel
