Migraine Abortive Management
- Rarely used in modern U.S. Migraine Management (largely replaced by Triptans)
III. Precautions
- Ergotamines have been replaced by Triptans for multiple reasons
- Low oral Bioavailability
- High risk for Medication Overuse Headache
- Common associated Nausea and Vomiting
- Serious adverse Drug Interaction risk
IV. Dosing
- Sublingual
- One tab SL
- Repeat in 1/2 hour prn
- Maximum: 3/day or 6/week
- Oral tablets available
- Rectal Suppository (Wigraine)
- One rectally at Headache onset
- May repeat hourly
- Maximum: 2 suppositories per day or 12 per month
- Limit no more than 2 days per week
- May use for Menstrual Migraine

- Use Antiemetic
- Rebound Headache
- Occluded vessel in Overdose
- Overdose levels
- Daily use exceeds 6 mg
- Weekly use exceeds 10 mg
- Potentiated by
- Beta Blockers
- Dopamine
- Fever
- Liver Disease
- Overdose levels
