| Definition (MSHCZE) | Střevní neprůchodnost, zástava průchodu tráveniny střevem. Závažný stav - druh náhlé příhody břišní, který může mít mnoho příčin. V zásadě se rozlišuje mechanický ileus, při němž je neprůchodnost způsobena mechanicky (překážka ve střevě, stlačení, volvulus aj.), cévní ileus vznikající při zaškrcení cévního zásobení střeva, trombóze či embolii a funkční ileus (neurogenní též paralytický ileus), který je způsoben „obrnou“ střevní svaloviny. K němu mohou vést některé léky, pooperační stavy, jiné břišní příhody aj. Biliární ileus je způsoben žlučovým kamenem, který pronikl do střeva ze žlučníku při jeho zánětu a vzniku píštěle. Kromě i. mohou být příznaky biliární, patrna může být pneumobilie. V patogenezi poškození se uplatňuje stagnace obsahu, poruchy resorpce a sekrece, pomnožení bakterií, hypoxie střevní stěny. Příznaky ileu jsou různé podle jeho typu a místa, v němž se průchod zastavil. Mohou být kolikovité nebo trvalé bolesti, zvracení, vzedmutí břicha při zástavě plynů. Rychlý nástup příznaků je zejm. u strangulace, při paralytickém ileu chybí zpočátku bolesti. Jako komplikace může vzniknout zánět pobřišnice peritonitida a šok ileózní nemoc. V diagnostice se uplatňuje fyzikální vyšetření břicha, nativní snímek břicha hladinky, akutní irigografie, ev. koloskopie při susp. ileu tlustého střeva, sonografie, laboratorní sledování. Stav vyžaduje hospitalizaci a intenzivní sledování, mechanický, cévní ileus a stavy se zánětem pobřišnice je třeba co nejrychleji operovat. (cit.
