- Anesthesia drug delivery on agents poorly soluble in water
- Lipophilic drug toxicity
- Calcium Channel Blocker Overdose
- Beta Blocker Overdose
- AntidepressantOverdoses
- Bupropion
- Cyclic Antidepressants
- Local Anesthetics that have been intravenously injected (esp. regional blocks)
- Other lipophilic agents
- Bromadiolone
- Diphenhydramine
- Flecainide
- Organophosphates
- Quetiapine
III. Mechanism: Lipophilic Drug Overdose
- Sequesters lipophilic drugs (forms micelle)
- Draws out lipophilic agents from aqueous and intracellular spaces
- Increases Cardiac Function (possible)
- Increases intracellular Fatty Acids triggering increased ATP in cardiac tissue
- Increases intracellular Calcium
IV. Dosing: Lipophilic Drug Overdose
- Intralipid 20% emulsion
- Bolus: 1.5 ml/kg (Lean Body Mass) IV over 2-3 minutes (may be repeated once)
- Maintenance (start immediately): 0.25 ml/kg/min IV
- Typically 15 ml/kg (Lean Body Mass) IV over 60 minutes
V. Adverse Effects
- Allergic Reaction
- Fat overload syndrome (severely lipemic)
- Hepatomegaly
- Splenomegaly
- Pancreatitis
- Seizures
- Fat embolism
