III. Criteria

  1. Respiratory Rate
    1. Score 0: Respiratory Rate <30/min
    2. Score 1: Respiratory Rate 30-45/min
    3. Score 2: Respiratory Rate 46-60/min
    4. Score 3: Respiratory Rate >60/min
  2. Wheezing
    1. Score 0: None
    2. Score 1: End-Expiratory Wheezes on auscultation
    3. Score 2: Inspiratory and Expiratory Wheezes on auscultation
    4. Score 3: Audible inspiratory and Expiratory Wheezes heard without a stethoscope
  3. Cyanosis
    1. Score 0: None
    2. Score 1: Perioral Cyanosis with crying
    3. Score 2: Perioral Cyanosis at rest
    4. Score 3: Generalized Cyanosis at rest
  4. Accessory Muscle Use
    1. Score 0: None
    2. Score 1: Mild intercostal retractions
    3. Score 2: Moderate intercostal retractions
    4. Score 3: Moderate to severe intercostal retractions with head bobbing or tracheal tug
