II. Pathophysiology
- Reaction occurs with treatment of Spirochete infection
- Syphilis (reaction occurs variably based on stage)
- Primary Syphilis: 50%
- Secondary Syphilis: 90%
- Latent Syphilis: 25%
- Relapsing Fever (Tick-Borne Disease)
- Syphilis (reaction occurs variably based on stage)
III. Signs and Symptoms

General
- Fever
- Adenopathy
- Skin rash
- Arthralgias
- Malaise
- Diaphoresis
- Neurologic symptoms
IV. Course
- Occurs 2 to 6 hours after treatment started
- Symptoms last for 24 hours
V. Management
- Acetaminophen 2 hours before and after Antibiotic dose
- Corticosteroids and NSAIDs do not prevent complications
- Maintain hydration
