II. Definition
- Patient attempts to maintain body position at rest (despite support against gravity)
III. Symptoms
-
Tremor characteristics
- Low to moderate frequency: 3 to 6 Hz
- High amplitude
- Patient attempts to maintain body position at rest
- Provocative factors (increases Tremor amplitude)
- Mental stress
- Distraction (e.g. Serial 7s, counting backwards)
- Moving another body part (e.g. walking)
- Palliative factors (decreases Tremor amplitude)
- Goal-directed movement (e.g. Finger-Nose-Finger)
IV. Examination
- See Parkinson's Disease
- Passively flex and extend arms for cogwheel rigidity
- Observe for shuffling gait
V. Causes
- Parkinson's Disease (most common)
- Multiple-Systems atrophy
- Progressive Supranuclear palsy
-
Drug-Induced Tremor
- Neuroleptic medications
- Metoclopramide (Reglan)
- Rubral Tremor
- Psychogenic Tremor
- Severe Essential Tremor
VI. Management
- Levodopa-Carbidopa (Sinemet)
- Anticholinergics
- Other antiparkinson Meds
- Deep Brain Stimulation
- Pallidotomy
- Thalamotomy
