II. Epidemiology
- Incidence: 2 per 1000 live births
III. Pathophysiology
- Underlying lung or systemic conditions (see below) prevent transition from Fetal Circulation
- Increased pulmonary pressures lead to right-to-left shunt and Hypoxemia
IV. Causes
- Meconium Aspiration Syndrome
- Neonatal Pneumonia
- Neonatal Sepsis
- Respiratory distress syndrome of the newborn
- Diaphragmatic Hernia
- Pulmonary hypoplasia
V. Risk Factors
- Gestational Diabetes
- Cesarean delivery
- Maternal Obesity
- Black race
- Maternal SSRI use (low risk)
VI. Diagnostics
VII. Signs
- See Neonatal Respiratory Distress
- Systolic Murmur
- Loud Second Heart Sound
-
Hypoxia
- Improves with Supplemental Oxygen
VIII. Management
- See Neonatal Respiratory Distress
- Consider N-CPAP or Endotracheal Intubation
- Consult neonatology
- Consider pulmonary vasodilators (Sildenafil, inhaled nitric oxide)
- ECMO has been used
