III. Complications: Conditions secondary to blunt Trauma

  1. Hyphema
  2. Retinal Detachment
  3. Eyelid Laceration
  4. Penetrating Eye Injury with Globe Rupture
  5. Lens Dislocation
  6. Traumatic Glaucoma
  7. Iridodialysis
    1. Traumatic separation of iris from ciliary body
    2. Risk of Aqueous Humor accumulation (blocked absorption) with Increased Intraocular Pressure
  8. Cyclodialysis Cleft
    1. Separation of ciliary Muscle fibers from attachment at Scleral spur
    2. Results when the globe is briefly compressed followed by rapid re-expansion
    3. Results in Aqueous Humor leak from anterior chamber into suprachoroidal space
    4. Presents with Ocular Hypotony (Intraocular Pressure <5 mm Hg)

IV. Imaging: Indicated if penetrating injury suspected

  1. Orbital CT Scan (1-1.5 mm cuts, axial and coronal) or
  2. XRay (up and down gaze views) if CT not available

V. Management

  1. Patients who are discharged
    1. Close interval follow-up
    2. Self-monitoring of Vision and immediate return for worsening or lack of improvement
