V. Dosing: Adults

  1. Daptomycin 4 mg/kg in NS over 30 minutes once daily

VI. Adverse Effects

  1. Muscle toxicity (avoid concurrent Statin use)

VII. Monitoring

  1. Obtain serum CPK level weekly while on Daptomycin

Definition (NCI) A semi-synthetic cyclic lipopeptide antibiotic isolated form the bacterium Streptomyces roseosporus with broad-spectrum antibiotic activity against Gram-positive bacteria.
