II. Pathophysiology
- Characteristics
- No Lancefield Group assigned
- Gram Positive Cocci in Pairs (Diplococcus)
- Alpha-hemolytic Streptococcus
- Virulence Factors
- Polysaccharide Capsule protects it from Phagocytosis
- With infection, antibodies develop to capsule, but only protect against one of 84 serotypes
- Vaccines are polyvalent (21 of the most virulent serotypes as of 2024)
- Infections
III. Labs
- Microscopy
- Gram Positive Cocci in Pairs
- Alpha-hemolytic Streptococcus (like Viridans Streptococcus)
- Green coloration on Blood agar plate (BAP)
- Differentiated from Viridans Streptococcus (also Alpha-hemolytic Streptococcus)
- Quellung Reaction (Neufeld reaction)
- Bacterial slide mixed with capsular antibodies and methylene blue
- Pneumococcal capsule swells in response to specific antibodies
- Bile solubility (10% deoxycholate)
- Streptococcus Pneumoniae dissolves in bile acids, while Viridans remains intact
- Optochin or "P" disc susceptibility (ethylhydrocupreine hydrochloride impregnated plate)
- Streptococcus Pneumoniae growth is inhibited by Optochin
- Quellung Reaction (Neufeld reaction)
