African Trypanosomiasis
- Preferred agent in stage 1 and 2 (age >=6 years and weight >20 kg)
III. Contraindications
- Age <6 years
- Weight <20 kg
- Unknown CSF Leukocyte count OR CSF Leukocytes >100 wbc/uL (severe Stage 2, Cerebral Trypanosomiasis)
- Severe hepatic Impairment or Cirrhosis
- Severe renal Impairment
- Cockayne syndrome
- First trimester pregnancy (may use in second or third trimester)

- Avoid in West African Trypanosomiasis
- However, relative contraindication due to rapid lethality of East African Trypanosomiasis
- Start with either Fexindazole or Pentamidine in pregnancy regardless of trimester
IV. Mechanism
- Fexinidazole is a nitroimidazole Antiprotozoal Agent
- Fexinidazole is a prodrug
V. Medications
- Fexinidazole 600 mg tablets
VI. Dosing
- Dosing weight >=35 kg
- Load: 1800 mg (3 tablets) once daily for 4 days
- Next: 1200 mg (2 tablets) once daily for 6 days
- Dosing weight 20 kg to 35 kg
- Load: 1200 mg (2 tablets) once daily for 4 days
- Next: 600 mg (1 tablet) once daily for 6 days

- Half-Life: 15 hours (in normal Renal Function)
VIII. Adverse Effects
- Serious adverse effects (uncommon)
- QT Interval Prolongation
- Neutropenia
- Hepatotoxicity
- Neuropsychiatric effects
- Agitation or anxiety
- Abnormal behavior or personality change
- Depressed mood or Suicidal Ideation
- Nightmares
- Hallucinations
- Common neurolopsychiatric adverse effects
- Common gastrointestinal adverse effects
- Nausea or Vomiting
- Decreased appetite
- Dyspepsia
- Upper Abdominal Pain
- Back pain
- Common Electrolyte abnormalities
IX. Safety
- Lactation
- Pregnancy
- May use in second or third trimester
- First trimester use
- Toxicity in rat and rabbit studies have been found well above typical dosing
- Avoid first trimester use in West African Trypanosomiasis
- However, may be used first trimester in East African Trypanosomiasis (rapidly fatal)
- Start with either Fexindazole or Pentamidine in pregnancy regardless of trimester
X. Drug Interactions
- Disulfiram reaction when combined with Alcohol
