- May offer rehabilitation, skilled nursing care, and adjunctive health and social services
- Includes various levels of care from SNF and TCU to Nursing Homes and intermediate care
- Skilled Nursing Facility (SNF)
- Facility offering continuous 24 hour nursing, with medical supervision and adjunctive health and social services
- Provides a level of care below acute hospital care and above intermediate care and custodial care
- Intended for rehabilitation to greater functional capacity in ADLs and self care (often as a TCU stay after hospitalization)
- Tailored services based on patient needs include physical therapy, occupational therapy, speech therapy

- Transitional Care Unit (TCU)
- Short-term care (typically up to 21-28 days) in a Skilled Nursing Facility

A nursing home is a place for people who don't need to be in a hospital but can't be cared for at home. Most nursing homes have nursing aides and skilled nurses on hand 24 hours a day. Some nursing homes are set up like a hospital. The staff provides medical care, as well as physical, speech and occupational therapy. There might be a nurses' station on each floor. Other nursing homes try to be more like home. They try to have a neighborhood feel. Often, they don't have a fixed day-to-day schedule, and kitchens might be open to residents. Staff members are encouraged to develop relationships with residents. Some nursing homes have special care units for people with serious memory problems such as Alzheimer's disease. Definition (HL7V3.0) | <p>(1) A skilled nursing facility is a facility or distinct part of an institution whose primary function is to provide medical, continuous nursing, and other health and social services to patients who are not in an acute phase of illness requiring services in a hospital, but who require primary restorative or skilled nursing services on an inpatient basis above the level of intermediate or custodial care in order to reach a degree of body functioning to permit self care in essential daily living. It meets any licensing or certification standards et forth by the jurisdiction where it is located. A skilled nursing facility may be a freestanding facility or part of a hospital that has been certified by Medicare to admit patients requiring subacute care and rehabilitation; (2) Provides non-acute medical and skilled nursing care services, therapy and social services under the supervision of a licensed registered nurse on a 24-hour basis.
