II. Indications

  1. Predicts mortality in Stevens-Johnson Syndrome (or Erythema Multiforme Major) or Toxic Epidermal Necrolysis
  2. Directs management of SJS and TEN

III. Criteria: Assign one point for each of the following 7 risk factors

  1. Age 40 years old or older
  2. Heart Rate 120/minute or higher
  3. Malignancy history
  4. Surface area detachment 10% or more
  5. Blood Urea Nitrogen (BUN) >28 mg/dl (>10 mmol/L)
  6. Serum bicarbonate <20 mmol/L
  7. Serum Glucose >252 mg/dl (or >14 mmol/L)

IV. Interpretation

  1. Score 0-1
    1. Mortality: 3.2%
    2. No specific management typically needed
  2. Score 2
    1. Mortality: 12.1%
    2. Threshold above which Intensive Care (or burn center) admission is recommended
  3. Score 3
    1. Mortality: 35.3%
  4. Score 4
    1. Mortality: 58.3%
  5. Score 5-7
    1. Mortality: >90%
