- Non-sulfhydryl Angiotensin Converting Enzyme (ACE) inhibitor
- Perindopril is a prodrug, converted to its active form Perindoprilat via hydrolysis
IV. Dosing
- Adequate 24 hour Blood Pressure control may require twice daily dosing

Hypertension
- Start 4 mg orally daily or divided twice daily
- Start at 2 mg/day in renal dysfunction or concurrent Diuretic use
- Target 4 to 8 mg/day
- Start 4 mg orally daily or divided twice daily

Coronary Artery Disease secondary prevention
- Start 4 mg orally daily for 2 weeks
- May increase after 2 weeks to 8 mg/day

Congestive Heart Failure (not FDA approved)
- Start 2 mg orally daily
- Maximum 16 mg/day
- Maximum 16 mg/day

Definition (NCI) | A non-sulfhydryl angiotensin converting enzyme (ACE) inhibitor with antihypertensive activity. Upon hydrolysis, perindopril is converted to its active form perindoprilat, inhibiting ACE and the conversion of angiotensin I to angiotensin II; consequently, angiotensin II-mediated vasoconstriction and angiotensin II-stimulated aldosterone secretion from the adrenal cortex are inhibited and diuresis and natriuresis ensue. |
