II. Definitions

  1. Plasmacytoma
    1. Discrete, localized mass of neoplastic monoclonal plasma cells
    2. Mass may be found within Bone Marrow or extramedullary
    3. Other than being localized to a single mass, Plasmacytoma cells are Identical to those in plasma cell Myelomas
    4. Plasmacytomas may also progress to Multiple Myeloma

III. Diagnosis: Solitary Plasmacytoma (IMWG 2024, all 4 criteria must be met)

  1. Biopsy-proven solitary bone or soft tissue lesion containing clonal plasma cells AND
  2. Normal Bone Marrow without evidence of clonal plasma cells AND
  3. Normal imaging other than primary Plasmacytoma (Skeletal Survey, MRI, CT) AND
  4. Absent end organ injury attributed to lymphoplasma cell proliferative disorder (CRAB Factors)
    1. Hypercalcemia absent
    2. Renal Insufficiency absent
    3. Anemia absent
    4. Bone lesions absent

IV. Diagnosis: Solitary Plasmacytoma with minimal marrow involvement (IMWG 2024, all 4 criteria must be met)

  1. Biopsy-proven solitary bone or soft tissue lesion containing clonal plasma cells AND
  2. Clonal Bone Marrow plasma cells <10% AND
  3. Normal imaging other than primary Plasmacytoma (Skeletal Survey, MRI, CT) AND
  4. Absent end organ injury attributed to lymphoplasma cell proliferative disorder (CRAB Factors)
    1. Hypercalcemia absent
    2. Renal Insufficiency absent
    3. Anemia absent
    4. Bone lesions absent
