Vaginal Contraceptive Ring

- Background
- Ethylene vinyl ring
- Diameter: 2 inch diameter
- Thickness: 1/8 inch
- Vaginal ring releases hormones at constant rate
- Ethinyl Estradiol 15 mcg/day
- Etonogestrel 120 mcg/day
- Technique: 4 week cycles
- Inserted into vagina on or before day 5 of cycle
- Ring left in vagina for 3 weeks
- Removed for 1 week for withdrawal bleeding
- Backup Contraception needed if ring is out for >3 hours
- Efficacy
- Failure rate: 0.65 pregnancies per 100 woman years
- Compliance is greater than 90%
- Adverse effects
- Overall ring is well tolerated in 96% of women
- Foreign body sensation in vagina
- White Vaginal Discharge
- Vaginitis
- Difficult intercourse with ring in place
- Ring may be expulsed from vagina
- Headache may occur
- Benefits
- Easy insertion and well tolerated
- Less break-through bleeding than Levonorgestrel OCP
- Normal withdrawal bleeding is however more common
- Avoids first pass through liver
- No effect on lipids
- Allows for low Estrogen (15 mcg Ethinyl Estradiol)
- No effect on Lactation

- Remove the patch and insert the vaginal ring on the same day (no overlap needed)
- Switch may occur prior to the scheduled removal of the patch

- Apply the Ortho Evra patch for 2 days before removing the ring
- Switch may occur prior to the scheduled removal of the ring
