III. Mechanism

IV. Dosing: Adult

  1. Oral
    1. Start: 25 mg orally daily
    2. Increase to 50 mg orally twice daily after first day
    3. Increase to 100 mg orally in the AM and 50 to 100 mg orally in PM
    4. Maximum: 200-400 mg divided twice daily
    5. Onset: 30 minutes
    6. Duration: 6 to 8 hours
  2. Intravenous
    1. Infusion concentration: 50 mg/ml
    2. Start: 0.5 to 1 mg/kg IV slowly over 20 to 30 minutes
    3. Maximum: 100 mg/dose
    4. IV Dose in 2023 costs $2500 in U.S.
    5. Onset: 5 minutes
    6. Duration: 2 hours

V. Dosing: Child

  1. Not FDA approved for use in children, and avoid in infants
  2. IV: 1 mg/kg IV
