Regular: 40 mg PO bid (MAX: 640 mg/day)
Extended: 80 mg PO qd (MAX: 320 mg/day)
Dosing
Intravenous
Load: 1-3 mg IV over 2-5 minutes
Repeat every 2 minutes to a total dose of 0.1 mg/kg
Sustained response for 6-8 hours
