  1. Coronary Artery Disease (66% of cases)
  2. Cardiomyopathy with reduced left ventricular ejection fraction
  3. Aortic Stenosis
  4. Electrolyte disturbance (especially drug induced)
  5. Viral Myocarditis
  6. Ventricular arrhythmias
