- Severe Fatigue >6 months
- Persistent or relapsing in course
- Does not resolve with bed rest
- Significant reduction in average daily activity (below 50% of prior activity)
- Other causes excluded

- Minor Criteria (4 or more present for 6 months)
- Headache
- New type, severity or pattern
- Typically non-focal Headache
- Migratory Polyarthralgias
- Non-inflammatory (no swelling or erythema)
- Myalgias
- Postexertional malaise or Fatigue
- Duration >24 hours
- Short-term memory or concentration impaired
- Pharyngitis
- Tender cervical or axillary adenopathy
- Non-restorative sleep

- Fatigue is primary symptom
- Timing
- Onset at a definite point in time
- Duration 6 months or greater
- Present at least 50% of time
- Severity
- Severe and disabling
- Affects physical and cognitive functioning
- Associated symptoms (at least one must be present)
- Myalgias
- Mood Disorder
- Sleep disorder
- Symptoms not due to alternative diagnosis
- Established medical condition known to cause Fatigue
- Established neurologic or psychiatric disorder
- Schizophrenia
- Bipolar Disorder
- Substance Abuse
- Eating Disorder
- CNS disorder

- Two or more signs must be documented
- Physician must document each sign twice
- Signs must be present for longer than 1 month
- Criteria
- Low grade fever (37.6 - 38.6)
- Non-Exudative Pharyngitis
- Palpable or tender Lymph Nodes (size <2 cm)
- Anterior or Posterior Cervical Lymphadenopathy
- Axillary Lymphadenopathy
