- Score 3: Respiratory Rate >60/min
- Wheezing
- Score 0: None
- Score 1: End-Expiratory Wheezes on auscultation
- Score 2: Inspiratory and Expiratory Wheezes on auscultation
- Score 3: Audible inspiratory and Expiratory Wheezes heard without a stethoscope
- Cyanosis
- Score 0: None
- Score 1: Perioral Cyanosis with crying
- Score 2: Perioral Cyanosis at rest
- Score 3: Generalized Cyanosis at rest
- Accessory Muscle Use
- Score 0: None
- Score 1: Mild intercostal retractions
- Score 2: Moderate intercostal retractions
- Score 3: Moderate to severe intercostal retractions with head bobbing or tracheal tug
